Rule 4765-20-05 | Notification to the executive director of change of personal information.
(A) The holder of a firefighter, fire safety inspector, or hazard recognition officer certificate issued pursuant to section 4765.55 of the Revised Code and this chapter of the Administrative Code shall provide written notice to the executive director no later than thirty days following any changes in the certificate holder's:
(1) Full legal name;
(2) Home mailing address;
(3) Personal telephone number;
(4) Personal email address;
(5) Affiliations.
(B) A certificate holder notifying the executive director of a name change shall submit legal documentation verifying such a name change.
