(A) All requests for injury leave must be filed within forty-five days of the original injury or last day worked for subsequent absences.

(B) In the event the officer is unable to make application for occupational injury leave due to the officer's health and medical condition, a member of the employee's family or representative of the employee may file and sign the occupational injury leave request.

(C) Certification of the occupational injury leave request will not be final until it has been approved by the Superintendent of the Ohio state highway patrol.
